What to Bring to Your Exam
A categorized checklist to ensure you arrive prepared with all required identification, paperwork, and immunization records.
1. Government Photo ID
Valid unexpired Passport, Driver's License, State ID card, or Military ID.
2. Vaccination Records
Childhood or adult immunization yellow cards, translated into English if from overseas.
3. Form I-693
Latest version downloaded from USCIS with Part 1 completed (do not sign Part 2 until instructed).
4. Relevant Medical Records
Documentation regarding major past treatments, prior positive TB tests, or chronic medical conditions.
5. Corrective Eyewear
Bring eyeglasses or contact lenses if worn for standard visual check.
6. Prescription List
A list of any daily prescription medications with dosages you are currently taking.
